Allegations

The complaint alleges that the employer failed to provide timely, compliant meal periods to non-exempt hourly employees and failed to pay premium pay for non-compliant meal periods. It further alleges that the employer failed to authorize and permit rest breaks to non-exempt hourly employees and failed to pay premium pay for missed rest breaks. It further alleges that the employer failed to pay minimum wages for all hours worked, including uncompensated off-the-clock work. It further alleges that the employer required employees to use personal cell phones and pay out of pocket for scrubs, masks, sanitizer and other work equipment without reimbursing necessary business expenditures. It further alleges that the employer failed to provide accurate itemized wage statements that reflect all hours worked, premium pay, overtime premiums and expense reimbursement.
